Problems with transparent proxy

I'm running Squid 2.2 stable 4 on RH 5.2 using transparent proxying w/ a
Cisco router.
When a client tries to access a web page on a virtual web server the Squid
server would return the home page of a different virtual server hosted on
the same machine, or an error message like "File not found."
However, after some time - I guess the cache entry expires - one would get
the right page.
I was running 2.1 before with the same symptoms.
I'd really appreciate your help.

PS. I've read the FAQ and searched the archive and I've only found the same
question :)
