Can't seem to get Squid caching properly

I been trying to get squid working for well over a week now, but it just
doesn't want to cache any objects, I'm constantly getting TCP_MISS/200
messages when the pages are in the cache. I've tried it with a number of
squid.conf files, and have resorted to using this basic one just to try
and get it working:

acl all src 0.0.0.0/0.0.0.0
http_access allow all
cache_effective_user squid squid

I tried both squid 2.2-STABLE3 and STABLE4.

My system is:

Slackware 4.0 Linux (i586), kernel 2.2.10, egcs 2.91.66.

Does anyone have any ideas? Have I missed something really obvious here?
