You can easily extend htpasswd+ncsa_auth with password aging if needed.

Yes, there exists a multitude of different authentication modules to
Squid. Most of them plug into other authentication shemes (like LDAP,
UNIX or PAM) and at least some of these other password shemes support
password aging.
