What is the output of
  client mgr:store_check_cachable_stats
(or "storeCheckCachable() Stats" in cachemgr)

async-io in Squid 2.3 has several shortcomings compared to Squid 2.2. My
recommendation for the time being if you need async-io is to use
squid-2.2.STABLE5-hno (see http://squid.sourceforge.net/hno/).
