Simon,
You will need to run the configure script in the top directory of the
squid source. This establishes all settings about your OS and compiler
and creates the approriate makefiles in all the relevant directories.

This is documented in the INSTALL file in the top directory of the
squid source.

Once you have done this, change to the ncsa_auth directory and type make.
This will then make the ncsa_auth module.
