AW: Failed to select source?

I had this problem, too. The reason was a dead parent and usually
shows of in /var/log/messages.

Jan 18 11:44:22 dns7 squid[778]: Detected DEAD Parent:
dns0.otto.de/3128/3130
Jan 18 11:44:22 dns7 squid[778]: Failed to select source for
'http://www.bmw.de/'
Jan 18 11:44:22 dns7 squid[778]: always_direct = -1
Jan 18 11:44:22 dns7 squid[778]: never_direct = 1
Jan 18 11:44:22 dns7 squid[778]: timedout = 0

solution was to increase performance between parent and proxy-server
and changing the dead_peer_timeout

dead_peer_timeout 600 seconds

Have a look at what was in the cache.log before and after you had these
lines. Usually I see a parent failure. You are forcing all requests
through a parent I take it.
