Hey, this is a "CLIENT_REFRESH_MISS". Note the word "CLIENT". Your browser asks
to force the reload of image when you hit "Reload" button. Every time you
hit this button, page will be fetched from original server, regardless on it's
presence in the cache. This is done, for example, if you see that page (picture)
looks bad adn want to retransmit it from original (just one example).
To see HIT, you can restart your browser and go to the same page. Squid should
display some sort of hit here. Or just walk on...
