Authenticate modules write the username automatically to access.log when you
used its in squid. i have seen username in access.log file when i have
installed NCSA and SMB_auth.

access.log native format like this (from FAQ):
"%9d.%03d %6d %s %s/%03d %d %s %s %s
%s %s/%s %s"
                                         ^ ^ ^ ^
                                        bytes req. method URL
rfc931
                                                 
The rfc931 define authentication server.
