Questions on "client mgr:idns" output

Below is the output of the command "client mgr:idns". This is on my
shiny-new Squid v2.3S4 installation. I've got a couple of questions
regarding this info.

1. Is anyone else seeing this misformatting? I'm guessing that there
is a missing newline after the text "ATTEMPT3".

2. Where the heck is "RCode Matrix" documented?
