This can be done via the redirector interface in Squid.

If the redirector rewrites the HTTP url to

http://user:password@www.acm.org/dl...

then Squid will automatically pick up the user:password part and
constructin a Basic HTTP authentication header when sending the request
to the server.

No modifications to the Squid source is required.
