Tony,

Did you compile fakeauth_auth when you compiled squid? If not, compile it,
if yes....
remove mydomain\server because it is not needed for fakeauth_auth.

domain\userid will show up in your access.logs.

as always, when you change squid.conf you need to /path/to/squidbin/squid -k
reconfigure
