Define "Registered user".

How is the cache supposed to tell if the user is registered or not? I..e
on which base is the registration tied to a user who then uses the
proxy.

If registration is "Per IP" then sure, it can be done with redirectors
to have requests from IP's not in the registration database redirected
to the registration page (make sure to have an exclusion for the
registration page in the redirection process).
