Upgrade to a 2.4 daily snapshot (or STABLE1...though it isn't as stable
as the snapshot), and enable the ie_refresh option.

I've also posted the patch for earlier Squids on my patches page. It
may or may not apply to 2.3STABLE4 (it was written for 2.2STABLE5+hno
and then ported forward to a 2.4DEVEL version).
