When you don't have a proxy-hierarchy (meaning your squid is a standalone proxy server) you don't have to log icp_queries because you want have any ICP_queries. ICP is the protocol used between proxies.
Another case where you don't have ICP is when you have for example a parent-proxy (firewall or ISP-proxy) who doesn't understand ICP (you would have defined this parent-proxy as no-query).
