The cachemgr password is only for the cachemgr functions, not proxying. You
also need to list which cachemgr actions this password applies to.

See the FAQ for how to set up authentication for using the proxy.

Have you perhaps disabled the client_db in squid.conf?

Check your client_db configuration directive setting.
