Well, I thought that the problem had gone away with the upgrade, but it has
resurfaced. It is no longer triggered by "reconfigure," random stale pages
will be served even if squid has not been "reconfigured."

Do you have any ideas what might be causing this? What debugging options
are recommended? Reading through debug-sections.txt it looks like I want
option 22, refresh calculation. Are there any other options that I want to
enable?
