Hello Erik,
The 1.2 patch should work with squid HEAD , I mean what you checkout
from the squid developer cvs at http://sf.net/squid - please try using
"patch -p1"

The squid-icap 1.0 patch was available for squid-2.4.STABLE2. We will
try and make the 1.2 patch available for squid-2.4.STABLE4 soon.

The easiest way to work with the ICAP enabled SQUID, currently, is to
either
* use the binaries (in .tgz files) of 1.2 release or
* checkout the icap enabled squid sources from the CVS repository
reachable at http://sf.net/icap-server.

Sorry if this sounds complicated..
We will try and simplify steps AND the documentation :-)
