It would be good if you could try using the new winbind helper. The
SMB/ntlm_auth helper is known to have it's rather serious quirks. The
winbind helper is a little bit more complex to install as you need to
install a Samba 2.2.4 or later as a domain member and with the needed
winbind support (see a recent message on Squid-dev from Andrew Bartlett
on the requirements).
