You create this file by using htpasswd or a similar tool. Remember
that Squid expects "crypt()" encoded passwords (some versions of
htpasswd supports multiple formats, see the htpasswd manual).

ncsa_auth is by default installed in
/usr/local/squid/libexec/squid/ncsa_auth ...
