If could also mean that the ntlm auth helper gets stuck and never
receives a response from the domain.. but in such case there should be a
URL logger after a long timeout.. (a minute or two).

Try running the helper from the command line and type

  YR

If the ntlm SMB helper can connect to the domain a

  TT <blob of data>

should be returned.
