Oh ok we are currently running the version below I will upgrade as suggested

Squid Cache: Version 2.5.STABLE4
configure options: --prefix=/usr --datadir=/usr/share --localstatedir=/var --sysconfdir=/etc/squid --infodir=/usr/share/info --mandir=/usr/share/man --enable-snmp --enable-ssl --enable-auth=ntlm,basic --enable-basic-auth-helpers=winbind --enable-ntlm-auth-helpers=winbind --enable-external-acl-helpers=winbind_group,wbinfo_group --enable-storeio=diskd,ufs

Which version of Squid-2.5? If less than 2.5.STABLE5+ntlm_assert patch
then upgrade as there has been identified multiple issues causing such
symptoms in earlier Squid versions.
