First the basic diagnostics:
0) Are you sure Squid isn't running?
1) Any warnings/errors from "squid -k parse"?
2) Any errors/warnings in cache.log when you attempt to start Squid?
3) Any errors on the console when you start Squid in foreground mode?
(squid -NYd1)
