we'd need to see your acl, http_access, http_reply_access and miss_access
settings...

it's in GMT. You probably don't have timezone configured on squid server.
(or in squid's chroot)

in such case I'd check what generates the error (parent proxy, intercepting
proxy in the way?) and would do it before checking for acl settings
