as predicted it's all the same with:

sbin/squid -v
Squid Cache: Version 2.5.STABLE12
configure options: --prefix=/opt/squid --enable-auth=basic,digest,ntlm

logs atatched.

and keep in mind: squid 2.4 works as expected, 2.5 is the problem.
