You should upgrade.. there has been quite many NTLM bugfixes in Squid in
the 3.5 years since 2.5.STABLE1 was released.

Yes your Samba needs to have winbind enabled. There is no special
configuration of winbind required I think and probaly why you don't find
any mentioned..

Are you sure your Samba server is not running winbind already? It's
normally enabled..
