I think this is a weirdness with the way squid allows deny_info to
override error pages.

I expect you will find the bogus-url deny is occuring after the point
where you set the deny_info page for clamsquid.

I found the solution to this was to explicitly reset the deny_info
destination after any custom changes like the one you have for clamsquid.
