Cool. installation of the patched version went fine (no errors after
'make deinstall && make reinstall'), but I'm still getting the errors
on startup:

2009/05/30 12:34:22| cache_cf.cc(346) squid.conf:40 unrecognized:
'header_access'
2009/05/30 12:34:22| cache_cf.cc(346) squid.conf:41 unrecognized:
'header_access'

These are lines in /usr/local/etc/squid/squid.conf:

header_access Unless-Modified-Since deny all
header_access Translate deny all

So, perhaps I'm not doing that correctly?
