ICAP server is TrendMicro IWSVA 5.x
squid version is 3.1.14

config to talk to ICAP:
icap_enable on
icap_preview_enable off
icap_service iwsva-icap-req reqmod_precache bypass=0
icap://[server]:1344/REQ-Service
icap_service iwsva-icap-resp respmod_precache bypass=0 icap://[server]:1344/resp
adaptation_access iwsva-icap-req allow all
adaptation_access iwsva-icap-resp allow all
