OK, I'll have a look at that setting.
short_icon_urls is at it's default (so it's on!)

Is this maby related to using c-icap via ICAP?

I'm installing from source:

root_at_proxy-cvk-1:/var/www/squid/icons/silk
contains a lot of icons, which are fairly old (2011!)

The sources contain the icons:
