I think with this last approach is too rigid and it won't be long before
somebody submits a patch to move those hard coded values back into the
config file.

My vote would be:
        - make default value for squid.conf variable "undefined"
        - if the actual value is undefined, use hard coded policy value
        - if the actual value is defined, use that value
