refcount allcoator

Has anyone reviewed this?

I completed it back in may - its a straightforward adaptation of the
cbdata code.

I'd like to suggest it for 2.5 inclusion - even if no code elsewhere in
squid uses it now.

Why?

Because there are repeated comments here that xyz should be refcounted -
and there is little point in having to merge two branchs to accomplish
that, when the allocator is both trivial and fundamental to such
activities.

I don't think having it in 2.5 poses any stability risk - as nothing
uses it.
