G'day Adam!
The missing swap file problem is fixed in 1.1.5.
To get a 'clean' log file written, delete the log-last-clean file (or what ever
it is called, my machine is dead at the moment :( ) before you restart Squid.
This will force Squid to rebuild the log file in 'slow' mode.
