Good question. Broken pipe is not a valid error code for disk writes.

When you see this, try running cachemgr to see what the FD is used for,
and check messages/syslog in case there is some problem with the
machine.

Try also enabling debug logging to get a clearer picture of what Squid
is trying to do ("squid -k debug" toggles debug logging on/off)
