If the documents are static and don't have explicit expiry
information, use refresh patterns.

If you want to override an explicit expiry date/ maximum age, start
looking at the parameters with warnings about violating the protocol.

One other thing. Flush the disk caches in all the browsers
regularly, to prevent them from making If-Modified-Since requests, as
otherwise, the cached file may get deleted and squid currently cannot
cache 304 responses so the browsers will maintain their cache by
doing end to end IMS responses and the cache will never reload.
