ICP carries very limited amount of information, and it may be possible
that the request is more stale than permitted by the requestor or child
cache. This situation should however result in a TCP_REFRESH_* and not a
TCP_MISS.

There is also a race condition where the objcet may have been purged
from the cache in between the ICP and HTTP requests.

Third, if you use prefer_direct off or never_direct, then the request
may be a forced refresh (no-cache) causing the parent to ignore it's
cache.
