How is it build?
On what OS?
To help finding out what the problem might be can you please get a debug
trace next time it happens:
/usr/local/squid/bin/squid -k debug ; sleep 2 ;
/usr/local/squid/bin/squid -k debug
(turns on debug logging for 2 seconds)
