This is most likely way to high setting of cache_mem. Your machine only
has 256MB of memory, and spending 128MB of this for in-transit and hot
objects leaves only 128MB for the OS, disk buffers, Squid metadata,
other processes and other important stuff. It is possible that this
pushes your machine out to swap.
I'd recommend setting it to something in the range of 8 - 32 MB.
