You can find out which ACL entry is allowing requests with
this technique:

        edit squid.conf, add '33,2' to debug_options:
                deubg_options ALL,1 33,2

        reconfigure squid
                squid -kreconf

        look at cache.log, you'll see stuff like this:

2000/07/05 14:39:33| The request GET http://www.pepsi.com/ is ALLOWED, because it matched 'xyz'
