Did you build Squid with dnsserver helper support enabled ? i.e.

  ./configure --disable-internal-dns

By default, Squid 2.3 uses the new internal DNS lookup/caching code.

With helper support enabled the limit is normally 32 processes, but
you can increase this by tweaking the value of DefaultDnsChildrenMax
in src/defines.h
