 It means that SQUID can not access the object, at that moment,
 in the cache dirs, and it believes it should be there.
 This can happen during a restart of SQUID when it rebuilds the
 store index, so in fact, is happy doing internal work, and does
 not want to be disturbed.
 
 Or , there is something wrong, with the access to your store,
 disk access, hardware/software/file protection...

 I think, that SQUID, in these case fetches the object directly.
