Henrik,

I might be wrong but here is what I found.
In helper.c:GetFirstAvailable
There is no check at helper.c:GetFirstAvailable for flags.shutdown and
if the helper is still alive and already flagged with shutdown flag,
squid will keep sending requests to it.
