Current supported version is 2.5.STABLE7, with 2.5.STABLE8 on it's way.
2.3.STABLE4 is over 4 years old and is known to have very many issues and

See the pam_auth manpage documentation included in the Squid distribution.
You probably need a newer version for up to date documentation.
