maybe you should set reload_into_ims on, and more
extreme configuration is to set all cacheable file to
ignore-reload.

for example:
refresh_pattern -i \.jpg$ 10080 100% 43200
reload-into-ims
refresh_pattern -i \.swf$ 10080 100% 43200
ignore-reload
refresh_pattern . 10 50% 43200 reload-into-ims

but i suggest you to block banners rather than
modifying refresh_pattern
