http://wiki.squid-cache.org/SquidFaq/SquidAcl is your best starting
place. Read both the "ACL elements" and the "Access Lists" sections. I
think the most relevant statement is...
> Access list rules are checked in the order they are written. List

Without knowing the order of your http_access rules, helping you is
going to be tough.
The FAQ also contains a section regarding common mistakes, and another
section about debugging ACLs. Self-help if you prefer it.
