I'm seeing the same issue in squid 3.1.4. request->client_addr seems to
be zero in Adaptation::Icap::ModXact::makeRequestHeaders but I have no
idea why.

I tried searching bugzilla but it seems to be broken atm, does anyone
know if this is has been raised as a bug?
