It happens on every connection. That means ~ every 10 requests in my
experience. The user shouldn't notice however.

cache.log may have some hints - is there anything unusual there. You
also haven't indicated your configuration - are you using fake_auth or
NTLMSSP ?

Have you looked in the squid-users archives? Several other users have
had this problem, and most have been resolved by recent work, much of
which is only available in the ntlm branch from sourceforge. NTLM is not
production ready unfortunately, we are working hard on getting it there.
