Odd.. never had any such problems ever.. I do this very frequently..
but I am not using the PAM module.

Maybe it is the PAM module that gets confused on who the user is in
your environment. Try using the oneshot mode of pam_auth 2.0
available from <http://devel.squid-cache.org/hno/software.html#PAM>
or in Squid-2.5..
