I also have some problems with the NTLM authentication (using
squid-head-200104232300), but our firewall doesn't allow CVS access to the
outside world. Is there a way to download a whole branch from CVS using the
web frontend, or is there some other solution ? Opening up ports on the
firewall is not an option :(

I'd like you to try the bleeding edge NTLM code and tell me if it fixes
this problem for you, - its accessed via CVS from squid.sourceforge.net.
The cvs branch (tag) you want is ntlm.
